Multidrug resistance (MDR) is a major global health challenge, often caused by the overexpression of bacterial efflux pumps. These pumps actively expel antibiotics, reducing their intracellular concentration and effectiveness. Efflux Pump Inhibitors (EPIs) block this mechanism, allowing antibiotics to work more effectively against resistant bacteria
This invention repurposes Montelukast, an FDA-approved drug, as an EPI to combat multidrug-resistant Staphylococcus aureus. Although Montelukast lacks direct antibacterial activity, it enhances the efficacy of antibiotics like fluoroquinolones and β-lactams by inhibiting efflux pumps
such as NorA, NorB, and AbcA. This combination results in increased intracellular drug accumulation, reduced MIC, and a significant decrease in bacterial load

- Demonstrated strong synergistic effects with antibiotics like norfloxacin, moxifloxacin, and cefotaxime, resulting in >4-fold MIC reduction and enhanced bacterial killing.
